The objective of the managed care networks of specialized healthcare providers and reference centers is to guarantee treatment, accompaniment and psychosocial support of high quality for all patients. As far as possible the care shall be close to the place of residence. The care networks organize themselves and ensure national connectedness with other actors.
The tasks of the managed care networks of specialized healthcare providers and reference centers center on delivery of care, research, education and continuing education in universities, setting up disease-specific treatment paths and guidelines together with the platforms for rare diseases, participation in disease-specific registers, participation with international reference networks and cooperation with self-help organizations.
The networks are organized in a disease-specific manner (if possible according to larger disease groups) and they encompass healthcare providers from the secondary and tertiary levels. Within the networks the university hospitals play a special role as they are responsible for research, education and continuing education as well as implementing the registers. Should the creation of a reference center be reasonable, it shall be decided in the national coordination. The platforms and the disease specific patient organizations are the partners of the managed care networks and reference centers.

The interdisciplinary platforms pursue different objectives: thanks to them, patients as well as healthcare providers have a place of information in case of an unprecise diagnosis or a rare “non-recognized” disease. In the case of a rare disease they also ensure a simplified financing of healthcare services.
When it comes to the tasks for the platforms, they are responsible for delivering the information regarding suspect rare diseases, with rare and non-recognized diseases or with complex rare diseases. Furthermore, the platforms are responsible for care management (coordination of therapies), patient counseling, networking with other platforms and with patient organizations. Additionally, they are responsible for internal continuing education of coworkers and together with specialized clinics, they oversee the competences for the current research projects.
The platforms are interdisciplinary and cover all possible diseases. They do not need to be large associations but must have their own staff. The accreditation is undertaken by the national coordinating office for rare diseases.

L’objectif des réseaux de prestataires de soins et de centres de référence est d’assurer aux patientes et patients une prise en charge thérapeutique ainsi qu’un suivi et un soutien psychosocial de haute qualité. Dans la mesure du possible, cette prise en charge doit être mise en place à proximité du domicile du patient. Les réseaux de soins s’organisent de manière autonome et offrent des liens internationaux.
Les tâches des réseaux de soins et des centres de référence concernent le traitement, la recherche, la formation de base et continue sur le plan universitaire, l’élaboration d’un guide de prise en charge spécifique à chaque maladie et de lignes directrices (en concertation avec les plateformes pour les maladies rares), la participation à la tenue d’un registre spécifique à chaque maladie, la participation à des réseaux internationaux de référence et la coopération dans l‘organisation des soins.
Les réseaux sont organisés en fonction des maladies (selon la possibilité, en fonction des groupes de maladies) et comprennent des prestataires de soins des niveaux secondaires et tertiaires. Les hôpitaux universitaires jouent un rôle particulièrement important à l’intérieur du réseau dans la mesure où ils sont responsables de la recherche, de la formation de base et continue ainsi que de la tenue de registres. Si la création d’un centre de référence pour une maladie spécifique ou un groupe de maladies semble pertinente, celui-ci sera soumis à la coordination nationale pour approbation. Les partenaires des réseaux de soins et des centres de références sont les plateformes et les organisations de patients pour les maladies spécifiques.

Avec les plateformes interdisciplinaires pour les maladies rares, les patientes et les patients ainsi que les prestataires de soins disposent d’une centrale d’information en cas de diagnostic imprécis et de maladie rare «négligée». Dans des cas de maladies rares, ces plateformes garantissent et facilitent le financement des prestations.
Les tâches assumées par les plateformes comprennent l’information pour le dépistage des patients soupçonnant une maladie rare, avec une maladie rare „négligée“ ou une maladie rare complexe. De plus, les plateformes se chargent du care management (coordination du traitement), du conseil aux patients, de la mise en réseau avec les autres plateformes et organisations de patients. En outre, elles s’occupent de la formation interne des collaboratrices et collaborateurs et sont – en concertation avec les cliniques spécialisées dans des organes humains spécifiques – responsables des connaissances concernant la recherche en cours.
Les plateformes sont interdisciplinaires et couvrent l’ensemble des maladies. Elles ne doivent pas être des unités de grande taille, mais elles disposent en règle générale de leur propre personnel. La reconnaissance se fait par la coordination nationale pour les maladies rares.

Das Ziel der Versorgungsnetzwerke aus spezialisierten Leistungserbringern und Referenzzentren ist es, den Patientinnen und Patienten eine qualitativ hochstehende Behandlung, Betreuung und psychosoziale Unterstützung zu garantieren. Soweit möglich soll diese Versorgung wohnortnah organisiert werden. Die Versorgungsnetzwerke organisieren sich selbst und gewährleisten die internationale Anbindung.
Die Aufgaben der Versorgungsnetzwerke und der Referenzzentren betreffen die Versorgung, Forschung, Aus- und Weiterbildung im universitären Rahmen, Erarbeitung von krankheitsspezifischen Behandlungspfaden und Guidelines in Absprache mit den Plattformen für seltene Krankheiten, Teilnahme an krankheitsspezifischen Registern, Teilnahme an internationalen Referenznetzwerken und Kooperation mit Organisationen der Selbsthilfe.
Die Netzwerke sind krankheitsspezifisch organisiert (nach Möglichkeit nach grösseren Krankheitsgruppen) und umfassen Leistungserbringer auf dem Sekundär- und Tertiärniveau. Innerhalb der Netzwerke nehmen die Universitätsspitäler eine besondere Rolle ein, indem sie für die Forschung, Aus- und Weiterbildung sowie für die Registerführung verantwortlich sind. Ist die Schaffung eines Referenzzentrums für eine spezifische Krankheit sinnvoll, so wird dies in der nationalen Koordination zum Beschluss gebracht. Die Partner der Versorgungsnetzwerke und der Referenzzentren sind die Plattformen und krankheitsspezifische Patientenorganisationen.

Die interdisziplinären Plattformen bezwecken verschiedene Ziele zu erreichen; dank ihnen verfügen die Patientinnen und Patienten sowie die Erbringer einer Behandlung im Falle einer unpräzisen Diagnose oder einer seltenen „nicht anerkannten“ Krankheit über eine Informationsstelle. Im Falle seltener Krankheiten garantieren sie zudem die vereinfachte Finanzierung der Leistungen.
Die von den Plattformen übernommenen Aufgaben betreffen die Information für die Anerkennung von Patientinnen und Patienten mit verdächtigen seltenen Krankheiten, mit seltenen „nicht anerkannten“ Krankheiten oder mit komplexen seltenen Krankheiten. Zudem befassen sich die Plattformen mit dem care management (Koordination der Therapien), der Patientenberatung, der Vernetzung mit anderen Plattformen und mit Patientenorganisationen. Zudem kümmern sie sich um die interne Ausbildung der Mitarbeiter/-innen und sind im Einvernehmen mit den spezialisierten Kliniken, die sich mit bestimmten Organen befassen für die Kompetenzen zu den laufenden Forschungen zuständig.
Die Plattformen sind interdisziplinär und decken die Gesamtheit der Krankheiten ab. Sie müssen nicht grosse Verbände sein, aber sie müssen eigenes Personal aufweisen. Die Beglaubigung wird durch die nationale Koordination für seltene Krankheiten ausgeführt.

L’obiettivo delle reti di prestatori di cure specializzati e i centri di riferimento è quello di garantire una presa a carico terapeutica di qualità per i pazienti, nell’ambito di un sostegno psicosociale. Nella misura del possibile questa presa a carico dovrebbe avvenire vicino al luogo di domicilio del paziente. Le reti si organizzano in modo autonomo e garantiscono il contatto con l’estero.
I compiti delle reti e dei centri di riferimento riguardano le cure, la ricerca, la formazione di base e continua a livello universitario, l’elaborazione di guide e percorsi di cura specifici a ogni malattia (concertati con le Piattaforme dedicate alle malattie rare), la partecipazione alla tenuta di registri relativi alle malattie rare, la partecipazione a reti di riferimento interazionali e alla cooperazione con organizzazioni di sostegno.
Le reti sono organizzate secondo le singole malattie (se possibile, altrimenti secondo gruppi di malattie) e includono i fornitori di prestazioni di livello secondario e terziario. Gli ospedali universitari giocano un ruolo particolare all’interno delle reti: sono responsabili della ricerca, della formazione di base e continua e della tenuta dei registri. Se la creazione di un centro di riferimento per una malattia o un gruppo di malattie è pertinente, essa è sottomessa per approvazione alla coordinazione nazionale malattie rare. I partner delle reti e dei centri di riferimento sono le piattaforme e le organizzazioni a favore dei pazienti specifiche ad ogni malattia.

Le Piattaforme interdisciplinari mirano a raggiungere diversi obiettivi; grazie ad esse i pazienti e i prestatori di cure dispongono di una centrale di informazione in caso di diagnosi imprecisa o di malattia rara “non riconosciuta”. Esse garantiscono inoltre la semplificazione dei finanziamenti delle prestazioni in caso di malattie rare.
I compiti assunti dalle Piattaforme riguardano l’informazione per il riconoscimento di pazienti con sospette malattie rare, con malattie rare “non riconosciute” o con malattie rare complesse. Inoltre le Piattaforme si occupano del care management (coordinazione dei trattamenti), della consulenza ai pazienti, della messa in rete con altre piattaforme e con organizzazioni che operano a favore dei pazienti. Inoltre, si occupano della formazione interna per i collaboratori e sono responsabili delle competenze concernenti la ricerca in corso, in concertazione con le cliniche specializzate che si occupano di organi specifici.
Le Piattaforme sono interdisciplinari e coprono l’insieme delle malattie. Non devono essere grandi unità ma disporre di personale proprio. L’accreditamento è effettuato attraverso la coordinazione nazionale per le malattie rare.
